A torn carrier, flattened fastening panel or cut-resistant glove with a split seam can look minor at handover. In a frontline setting, it may be the point at which equipment no longer performs as expected. The discipline to record PPE damage at the time it is found protects the individual wearing it, gives supervisors a clear decision trail and supports an employer's duty of care.
For security teams, hospital staff, council rangers, corrections personnel and other workers exposed to foreseeable edged-weapon risk, inspection records should not be treated as paperwork for its own sake. They are part of a practical control system: inspect, identify, isolate, assess, replace and document. When that system is consistent, unsafe equipment is less likely to return to service by mistake.
Why recording PPE damage is an operational control
PPE is often used across long shifts, changing environments and multiple users. It is exposed to friction, sweat, cleaning agents, vehicle seats, sharp edges, crowded spaces and repeated fastening cycles. Damage can develop gradually, particularly where equipment is stored poorly or worn under operational pressure.
A verbal report is not enough. Details are easily lost between shifts, and a supervisor who was not present cannot reliably judge what occurred, where the item is now or whether it was removed from use. A written record creates accountability and allows the organisation to identify repeat issues, such as a particular storage method causing abrasion or a sizing problem placing excess strain on closures.
The record also matters after an incident. If equipment has been involved in a confrontation, a visible mark may not show the full extent of the impact. The correct response is not to make assumptions based on appearance. Isolate the item, document the circumstances and have it assessed in line with the manufacturer's instructions and your organisation's procedure.
What counts as damage or a defect?
Damage is not limited to a dramatic tear or puncture. Any change that could affect fit, retention, coverage, protective performance or safe use should be reported. For a protective vest, this may include a cut, puncture, crushed area, deep abrasion, exposed protective material, distorted shape, damaged stitching, failed zip, loose hook-and-loop fastening or a carrier that no longer holds the protective panels correctly.
For gloves and operational apparel, look for splits, worn-through areas, torn seams, contamination that cannot be safely removed, damaged cuffs and loss of grip or dexterity. A damaged radio loop or accessory attachment may appear separate from the protective function, but it can still create a snagging risk or interfere with how the equipment sits on the wearer.
Fit changes deserve attention too. If a vest has become difficult to secure, shifts during movement or no longer provides the intended side coverage, do not simply adjust around the problem. Record it. The cause may be wear, incorrect sizing, a changed wearer profile or a carrier issue. Each requires a different remedy.
Distinguish wear from damage, but do not minimise either
Normal surface wear does not always mean an item is unsafe. Light scuffing on an outer carrier, for example, may be cosmetic if all closures, seams and protective components remain sound. However, the person completing the inspection should not be asked to make technical judgements beyond their training.
Use a simple rule: if there is doubt about whether the item remains fit for service, treat it as suspect until an authorised person has reviewed it. This is especially relevant where protective materials have been cut, punctured, heavily compressed, exposed to extreme heat or chemicals, or involved in an assault.
How to record PPE damage properly
A useful damage report is specific enough that another person can find the item, understand the concern and make a defensible decision without relying on memory. It should be completed as soon as practicable after the damage is found or reported.
Start by identifying the equipment. Record the item type, brand or model where relevant, asset number, serial number, size and the wearer or team it was issued to. If an item has no asset identifier, introduce one. Fleet-level PPE is difficult to manage when several visually similar items cannot be distinguished.
Then describe the fault in plain, factual language. Write what is visible and where it is located. “Two-centimetre tear in the lower left carrier seam” is more useful than “vest damaged”. If the issue involves the protective component, avoid trying to diagnose whether it is still safe. State the observed condition: “deep abrasion and crease to front protective panel following restraint incident”.
Your record should include the date and time discovered, the location, the activity occurring at the time and whether an incident was involved. Add clear photographs where your policy permits. Take one image showing the whole item for context and close images of the affected area. Photographs support the report, but they do not replace the written description.
Finally, record the immediate action taken. This is the line that prevents an unsafe item being issued at the next shift. State whether it was removed from service, tagged, placed in a designated quarantine area, sent for assessment or replaced. Include the name of the person who made or approved that decision.
A practical process for supervisors and team leaders
Consistency matters more than complexity. A short form in a controlled digital system or a paper register can work, provided staff know where it is, complete it and supervisors review it promptly. The process should make it easy to do the right thing during a busy shift.
A sound procedure generally follows five actions:
- Inspect equipment at issue, before use and after any relevant incident.
- Stop using equipment where damage, contamination or performance is in doubt.
- Tag and physically separate the item from serviceable stock.
- Record the condition, circumstances, photographs and immediate action.
- Arrange authorised assessment, repair where permitted, or replacement, then close out the record.
When replacement is the safer decision
Not every damaged item should be repaired. Outer carriers, removable accessories and some garment components may be replaceable where the manufacturer permits it. Protective components are different. A cut, puncture, deformation or damage following an edged-weapon event requires a cautious approach and should be managed according to the manufacturer’s care, inspection and warranty guidance.
Cost pressure can lead organisations to keep marginal equipment in circulation. That is a false economy when the equipment is intended for workers facing a credible workplace hazard. The right decision depends on the type and location of the damage, whether the protective function may be affected, the item’s service history and the manufacturer’s assessment requirements.

A clean, timely record does more than satisfy an audit. It gives the next wearer confidence that the equipment issued to them has been checked, controlled and treated with the seriousness their role demands. That is the standard frontline teams deserve.
